Can't get the Houselinc software to recognize this device. Read "New Powerlinc 2413U problems connecting " here and grabbed the latest device file.

I'm thinking it might be defective. Sometimes, the LED is OFF. Frequently it's steady orange/red. Only ONCE it just constantly flickered green. Tried holding down the set button while plugging it in - no sound at all. So it seems I can't even reset it.

HL is legacy software and no longer supported. Any device is likely to not be recognized. Also, if the LED is not lit at all, then that's a sure indication of a defective device.

I would second the faulty PLM. When the LED is acting so strange.
Is it a brand new 2413U?

New from Newegg back in February. Not shrink wrapped of course so there's no way to tell for certain.

No Insteon devices have ever been shrink wrapped. IMO, Newegg is a reputable company.

I don't know if they'll honor the SH 2 year warranty, but it's worth a try

Does this thing run hot? Left it plugged in over night to see if it would "come alive" and the front is fairly warm but the other side with the label is too hot to touch for more than 2 seconds.

Newegg only does 30 days - officially. So I sent Insteon a note - wasn't sure what the warranty was.

Warm on the back is no issue.
Too hot to touch for more than a few seconds. Sounds like it has failed.
Warranty from Insteon is two years but may depend on Newegg being an authorized vendor.
Unless they fell off. There should be some white labels on the back of the 2413U. One of them will have a number on it with a date code and hardware revision. That could give you a rough idea how old it is.
Any hardware revision below 2.0 has the problem power supply components in it.

Guessing that's week 17 of 2015.

It's got a Smarthome label. Wonder if Smarthome warranties it or Insteon?

The date is correct. Smarthome is the retailer for Insteon. Any warranty issue should go through SH.
