I have the following switches that I am
trying to use in a 4 way setup.

*all are leviton switches (edit)
DD0SR-DL (3 way switch)
DW15S (4 way switch)
DD00R-DL (3 way switch)

DD0SR-DL >>>>> DW15S >>>>> DD00R-DL
The two 3 way switches are at the end of the light switch circuit and the 4 way switch is in the middle.

The lights should be connected to one of the 3 way switches, I think, in this case the DD00R-DL.

I get the following green light pattern on the switches.

DW15S normal blinking then off after about a minute
DD0SR-DL super fast blinking
DD00R-DL fast blinking

I can't seem to get these wired up correctly and do not know what the flashing green lights mean. Any guidance on how to continue would be much appreciated.

Thanks,
DV

What switches are you using? Insteon, Z-Wave?
None of them are still the original mechanical switches are they?

Is yours the wifi version? As best I can tell, the DW15S switch is the one with the brains and the one that must be connected to the load via red screw on switch. According to the instructions, when it is first installed and powered, it will blink a while indicating ready to be "configured", including adding to the wifi network.

I just installed a couple of Lutron Homekit switches (I don't recall the specific model number) and flashing LED indicated waiting to be added to the home app (or Leviton app). I remember that the switch came with a second set of instructions which described the incorporation of the switches into the app/IPAD. I don't recall that the three-way switch I installed even had an LED indicator.

Based on the instructions, The DD0SR-DL and DD00R-DL are the remote switches, used in conjunction with one of the smart decora switches such as the DW15S. I am no expert, but it appears one is an ON/OFF switch, the other has a dimmer control. Both include three connections, hot, neutral, and traveler, plus ground. None include a connection for the load.

Do you have neutrals in all your boxes (and how can you tell)? Have you identified unswitched hot anywhere? Have you identified the load conductor?
