I am positive that my 2475d is dead.

It appears that the replacement part is 2475DA1.
It looks like it still supports X10 and woudl be even dual.
However, in my setup I need the receptacle hinges to attach it to box. It looks like the new one does not have the "hinges" anymore?

It does look like the newer version has removed the mounting tabs. To mount it to the electrical boxes mounting ears.
Off hand I can't think of a way to do it. Hope others may chime in for you.

Use a 2477d. Just remove the faceplate.

BTW. I opened my dead 2475D. There is brown residue over some capacitors.
Unfortunately, it appears that all my 2475D look the same. Some still functioning.

Some of that is probably the semi-clear component glue they use to fastened the capacitor to the board for a secure mounting. It seems to get brown with age and internal temperatures. I have that in many of my older Insteon Modules.

Can you see any bulging on the capacitors or the rubber seals on the bottom leaking or popping out?

I would suspect the 470uF/35V capacitor in your photo.
I had the same value capacitors fail in some 2456S3 ApplianceLincs. One started pulsing On and off at a 60 cycle rate.
I have also had a 30 Volt Zener diode short in a 2456Ss.
